Injecting 30ccs of fluid under the fascia iliaca dissects laterally to get the lateral femoral cutaneous nerve in addition to the femoral nerve. Using like 10ccs only blocks the femoral nerve. Same injection site, different volume.

The patient was pretty obese and the parasacral block is not a block that I’ve done many times, so my first and second thought was that I am just not finding the nerve, as in the past I’ve obtained a strong sciatic twitch for confirmation.

It seems much easier to just have the surgeon inject pericapsular local directly during surgery than to muck around with ultrasound for a just OK block. I tried fascia iliaca blocks briefly and felt like it was 50/50 whether it provided any appreciable difference.

If the anesthesiologist works in orthopedics and doesn’t know how to do a fascia iliaca, PENG or LFCN block I would be concerned and I would bring that up with the surgeon and the anesthesia provider.
