I write BIRP notes in a school-based setting and have the 5 minutes between the school bells to write them. I’ve gotten good at “bookmarking” things mentally throughout the session so I know what I’ll write. I summarize client report in 3-4 sentences, my interventions in 3-4 sentences, their response in 2-3 and plan is 1 sentence.
Can anyone give me tips on writing a BIRP counseling note?

B: anxiety, poor social skills, auditory hallucinations I: Outreached client for group activity in the community. Introduced client to others in attendance. Role modeled appropriate social behavior.

Ask coworkers if you can see their notes to get a better idea. In my work I can see others’ notes (medical SW), and if I like the way another SW or psychiatrist phrased something I have a doc where I jot down good vocabulary/clinical language. Of course I leave out health info and identifying details.

2023年1月11日 · It's the only time I write notes during sessions--doing the genogram/history. That's the task in sessions 2-3. Those notes are later scanned to pdf and destroyed. I regard them as personal clinical notes. My notes are kept in Word, using a template I designed many years ago. The template is just a table in Word, with each cell a date of service.
